Family Faith Formation Registration 2020-2021

Welcome to our registration page for our new family faith formation venture! If you'd like more information on what exactly this new venture is then click HERE to learn more.

This program is open to the whole parish for anyone desiring growth, connectedness, and conversion in our Catholic faith. It also fulfills religious education requirements for Sacramental Prep and Confirmation.

Please fill out the form below to officially register your family.
Indicate which age group track you (and your family) will be attending. We have 3 tracks: Elementary: families primarily with children ages 6-12; Teen: families primarily with children 13-18; and Adults: individuals or couples ages 18 and up.
Each group will meet once a month inside the church as check-in, social distancing, and sanitation systems are already in place.
For those who don't yet feel comfortable to attend in person, Live Streams will be made available.
Gatherings will take place Wednesday evenings. Schedules will be announced soon.

This program will start October 21, 2020 OR October 28, 2020 (depending on your track).

To best meet the needs of our parishioners, we are offering events targeted to specific ages (in person & virtually). We understand many families have children whose ages fit in multiple tracks. Our goal this year is to keep families together, so the whole family will be encouraged to attend the events together we just want you to know that the content would be most developmentally appropriate for these age groups.

Important Notifications

Photos, Videos, & Livestream Notice	Photographs and/or videos may be taken during faith formation activities and events to be used for educational and public relation purposes. Names and addresses WILL NOT be included with any image or photograph. To disallow a photographic or electronic image of your child participating in a faith formation activity and/or event, you must notify Brita Jaingue in writing before you sign up to attend a live event. Please note all live events and activities will be livestreamed and we cannot guarantee that anyone who is present will not appear in the livestream and/or the recording.
Covid Release	Please note that anyone who attends one of our live events or activities will need to complete a waiver of liability relating to coronavirus/COVID-19.
Liabililty	As head of household and parent or guardian of my son/daughter, I hereby certify that I have read the above and I do hereby agree to allow myself and/or my son/daughter to participate in faith formation programs at St. Vincent de Paul Catholic Church. I knowingly and voluntarily, on behalf of myself and my minor child, do hereby agree to forever release, hold harmless and indemnify the staff and volunteers of these programs from any liability, claims, causes of action, loss, damages or injury to property or person arising out of my child’s participation in these programs.
